Add LTI columns to Institution

• Add lti_secret, lti_consumer_key, and lti_custom_inst_id fields to institution
• Use the lti hash in legacy/door data to populate those fields during migration
• Add to forms/show page
• Add to solr indexing configuration as indexed, stored fields e.g., string :field, stored: true
